Thank you for your interest in the work goals and work attitudes
study. On this web page is an informed consent form that describes the purpose
of the study and your participation. Once you click the "I Agree" box, you
will be asked to log in. To log in, please type your last name (all in lower
case) as your user name and the last 4 digits of your social security number
as your password (this ensures that two people with the same last name are
uniquely identified). You'll then find the survey, which takes 15-20 minutes
to complete. Finally, there is a space to provide your home address where
we can send a very brief survey for your significant other and a follow-up
survey for yourself (and eventually, your check). Please rest assured that
these surveys (for your significant other and the follow-up for yourself)
are very brief (5 minutes or less) and should not be onerous to complete.

INFORMED CONSENT FOR WORK GOALS AND WORK ATTITUDES STUDY
You are invited to participate in a research study on the relationship of work goals to work attitudes. You were selected as a possible participant because your are registered as a participant with studyresponse.com. Please read this form and ask any questions you may have before agreeing to be in the study. This study is being conducted by Tim Judge, Matherly-McKethan Eminent Scholar of Management, University of Florida, and Amy Kristof-Brown, Assistant Professor of Management and Organizations, University of Iowa.
The purpose of this study is to learn more about the relationship of work goals to work attitudes.
If you agree to be in this study, we will ask you to do the following things:
1. Complete an on-line survey about your work goals and job attitudes that will take approximately 20 minutes to complete.
2. There also is a very brief survey for a significant other to complete. This survey will be returned directly to us, and is confidential. (To preview this survey please click on Significant Other Survey.)
3. In several months, you will be asked to complete a final, very brief follow-up survey about work goals and attitudes. (To preview the follow-up survey please click here: Follow-up Survey.)
You do not have to answer any question you do not wish to answer. The ratings
you provide will be combined with those of other participants in this research.
Risks and Benefits of Being in the Study:
Participation in this study entails no more than minimal risk. There may be no personal benefit to your participation.
Costs and Compensation
There are no costs to you for being in the study. In return for participation, you will receive a $10 payment after the completion of the study.
Records of participation in this research project will be kept confidential to the extent permitted by law. Your surveys will contain a code number that is matched to your name so we can track your responses, and pay you when you are finished. The file containing your name and a code number assigned to you will be deleted once the study is finished. The electronic database of responses will not identify you in any way. However, federal government regulatory agencies and the University of Iowa Institutional Review Board (a committee that reviews and approves research studies) may inspect and copy records pertaining to this research. It is possible that these records could contain information that personally identifies you.
Voluntary Nature of the Study:
Participation is completely voluntary. If you decide to participate, you are free to withdraw at any time.
